Richland County restaurant inspections: critical violations July 25

The following restaurant inspections with critical violations were conducted between July 25 and July 28: ● Olive Garden Italian Restaurant #1390 , 558 Lexington Springmill Road, Mansfield, July 25.

The following restaurant inspections with critical violations were conducted between July 25 and July 28:

Olive Garden Italian Restaurant #1390, 558 Lexington Springmill Road, Mansfield, July 25. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations. Multiple critical violations observed (critical repeat). Correct by Aug. 4. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical). Employee along cook line indicated utensils in contact with TCS (time\temperature controlled for safety) foods and stored at room temperature (chicken slicer, fry batter bowl\colander, etc.) are changed out or washed\rinsed\sanitized only at shift change. Employee also indicated one shift can sometimes last up to 5-6 hours with no cleaning of the noted equipment in between. Employee unable to verbalize proper cleaning procedures for utensils and equipment. Education provided regarding ensuring utensils and equipment that are in contact with TCS foods and stored at room temperature are cleaned at least every 4 hours. Also observed employee improperly use food thermometer without sanitizing before or after use. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Observed employee took temperature of chicken on the grill without sanitizing thermometer before or after. Employee then placed soiled thermometer back into holder. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed TCS foods along cook line and at servers areas (shredded mozzarella, cut lettuce) held above 41 degrees for undetermined amount of time. Also observed containers of milk (half & half, chocolate miller, whole milk) in small cooler in south servers' area held above 41 degrees for undetermined amount of time. Observed whole milk in bar area to have internal temperature of 44 degrees. PIC or employee discarded all listed foods. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed tray of chicken parmesan in fryer cooler date marked for discard of July 24. Also observed slices of mousse cake in cooler date marked for discard on July 24. PIC voluntarily discarded items.

Kroger Company #N836, 1500 Lexington Ave., Mansfield, July 26. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). In the cheese cutting area, observed whole cheese to be sitting on a counter for 1-2 hours while employee was not at station. Cheeses did not have proper means of temperature control and internal temperature was 46-45 degrees. Discontinue practice of allowing TCS items to sit out without temperature control. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Handwashing sink is being used for purposes other than handwashing (critical, corrected). Observed remnants of food production in the hand sink in the produce area. Handwashing sink not accessible (critical repeat, corrected). Observed handwashing sink in the sushi area to be blocked with rolling push cart. TCS foods not being cold held at the proper temperature (critical, corrected). Observed whole cheese to be sitting on a counter with internal temperature of 46-45 degrees. Presence of live insects (critical repeat). During re-inspection, observed small flying insects in the deli area over the left three-compartment sink on and near the ceiling. Facility is to remove all evidence of insects and contact a licensed pest control operator to have service completed by re-inspection Aug. 2. Food contact surfaces not easily cleanable (critical repeat, corrected). Observed jalapenos, kimchi and other foods to be sitting on or wrapped in paper towels, rendering the food contact surface no longer smooth, durable, non-absorbent, and easily cleanable. Discontinue use of paper towel as a food contact surface. Insufficient air gap between the flood rim and the water supply inlet (critical repeat, corrected). In the sushi area, observed plastic bag tied around the air gap at the prep sink.

Porky's Drive In, 811 Ashland Road, Mansfield, July 26 & 27. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical). No chlorine registering on test papers. Make sure dish wash machine is functioning correctly; will recheck within 24 hours. Presence of live insects, and other pests (critical). Observed flies during inspection. Provide professional fly control measures.

Mansfield Fuel & Food Mart LLC, 3880 W. Fourth St., Mansfield, July 27. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed half & half, French vanilla creamer and American cheese not date marked. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed spray bottle of industrial cleaner not labeled.

Wiener King/Sunset Fast Foods Inc., 118 Lexington Ave., Mansfield, July 27. PIC unable to demonstrate knowledge by having no critical violations (critical). PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC could not state proper cooling parameters and methods. Provided education at inspection. Food employee(s) did not wash hands when required (critical, corrected). Observed employees moving from one task to another without proper handwashing or glove changing. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed package of hot dogs in the walk-in cooler without a covering. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed blade of can opener to have dried visible residue. TCS foods not being cold held at the proper temperature (critical, corrected). Observed American cheese, Swiss cheese, mayo, cut lettuce and uncooked bacon to have a internal temperature of 44-50 degrees on the make line next to grill. All TCS items were taken to the freezer to rapidly cool to 41 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed open container of milk in the walk-in cooler without a date mark. Presence of live insects (critical). Observed flying black insects throughout the facility, specifically front line, warewashing area and the dry storage areas. Contact licensed pest control operator and remove all evidence of flying insects by re-inspection Aug. 2.

Rancho Fiesta, 1360 S Trimble Road., Mansfield, July 27. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Presence of live insects (critical repeat). Observed small black flying insects near the mop sink, dry storage, and tortilla dry storage room. During second re-inspection, observed no change. Correct by Aug. 4.

Tamara K. King, 4218 Park Avenue West, Mansfield, July 27. Handwashing sink not accessible (critical repeat). Observed hand sink in bar area leaking and not fully functioning, rendering it inaccessible to food employees. Correct by Aug. 4. Drinking water not from an approved source (critical). Observed facility using a large water tank placed in the back of a pick up truck. PIC states she believes the owner of the building is filling the water tank from his home. Discussed ensuring that the water is being hauled in by a registered water hauler. Correct immediately. No air gap or approved backflow prevention device on the plumbing system (critical). No backflow observed on plumbing system for facility. Correct by Aug. 3.

Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed open package of granulated sugar on top of the microwave. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed orange juice opened days prior to be unlabeled.
